Q: does it have a AC97 header to plug in the computer case front headphone and mic ??

I can confirm that this Creative Sound Blaster Audigy FX PCIe 5.1 Sound Card does. I just installed this last week (Nov. 2019) and it is a perfect replacement to my old Soundblaster X-fi (SB0770) (PCI) sound card that had the connection to the front panel of my 2008 HP Pavilion Elite M9200t PC. As another responder mentioned-you can see the metal plug connectors right above the Soundblaster logo on the top of this sound card. That is where you connect the cable/cord coming from the front of your PC.

Q: Will the software (Audigy Fx Control Panel) allow me to adjust right/left balance to compensate for my hearing loss?

Depending on the number speakers you may use, most of us have two, left and right for stereo, you should be able to compensate for the issue mentioned from the audigy control panel. I have three speakers with a bass added and it took some playing with the controls to get the sound I wanted. It gives you an option for speaker/headphone configurations from stereo to 5.1 surround sound and one for headphones. Your mileage will vary. Hope this helps.

Q: can I run the 7.1 sound card with 5.1 speakers? I have 5.1 speaker set at the time but might upgrade to 7.1 speakers at a future date, would prefer to get the 7.1 card so I can upgrade later, but if it wont work with 5.1 speakers then its not going to work for me

Yes, a 5.1 speaker set up will work with a 7.1 card. A 7.1 speaker set up would just have one more cord to plugin than the 5.1 so it's very simple to upgrade your speakers later

No, this will not be useful for USB headsets, only items that are plugged into the card itself.
